ADVANCE HEALTH CARE DIRECTIVE (AHCD) & POLST - Tuesday June 3, 10:00 am Advance planning is the best way to ensure your wishes are honored and alleviates the worry of others about acting on your behalf. This presentation will provide up-to-date information on options for making your wishes known through an Advance Health Care Directive (AHCD), including the POLST (Physician’s Order for Life-Sustaining Treatment) which is becoming the standard in health care and community settings.
